This facility is provided for existing patients of Ivan Cammack (Optometrist) who wish to re- order Contact Lenses that have been supplied by the practice in the past.

Contact Lenses can only be supplied where we have a 'Contact Lens Prescription' which is less than 12 months old.

By making this request for a Contact Lens Supply you must accept that we will check your clinical records to ensure that you have attended for Contact Lens Aftercare within the last 12 months. If your Contact Lens prescription has expired, our practice staff will contact you to make the necessary arrangements.
